The Documents required for Passport Surrender:

  1. Application Form – Duly filled and signed

  2. Indian Passport – Copy of the first and last page

  3. Proof of Citizenship in Foreign Country

    • If you became a citizen after 2010: Attach a Citizenship Certificate

    • If you became a citizen before 2010: Attach a Citizenship Certificate
      OR a Citizenship Card (if certificate is not available)

  4. Foreign Passport Copy

    • Attach a copy of your Canadian or other foreign passport

    • If you hold a non-Canadian passport, include proof of legal status in Canada (e.g., PR card / Work Permit / Study Permit)

  5. Canadian Address Proof

    • Any one of: Driving License / Government-issued Photo ID / Utility Bill
