OCI miscellaneous application
An OCI cardholder who obtained the card when they were 20 years old or younger only needs to have it re-issued once after they receive a new passport once they turn 20.

Documents required for OCI Misc:
Dully filled application form.
Declaration by the OCI applicant.
OCI card original along with first and last page copy.
Canadian / Foreign passport copy.
In case applicant holds passport other than Canada, attach Canadian status ( Study permit / work permit / PR card).
Citizenship certificate copy.
Marital status proof
If married, provide marriage certificate copy.
If divorced, provide divorced certificate copy.
Spouse passport copy, if married
Employment letter
Self declaration will not be accepted
Latest employment letter is must
In case of not having employment letter, you can provide company offer letter under your name along with current pay stub
Incase the person is unemployed. On the application form under occupation section choose others and mention Unemployed. Next, give a self-declaration explaining why you are jobless.
If business / Self employed. Applicant need to attach article of incorporation.
Address proof
Driving license/utility bill/Notice of assessment/ Photo ID
Checklist
(2' x 2') one photograph.
