OCI Card Damage

Documents required for OCI Misc | Lost OCI :

1. Dully filled application form.

2. Declaration by the OCI applicant.

3. Self Declaration, how OCI card was damaged.

4. OCI card original and first and last page copy.

5. Canadian / Foreign passport copy.

   • In case applicant holds passport other than Canada, attach Canadian status ( Study permit / work permit / PR card).

6. Citizenship certificate copy (if applicable) / citizenship card.

7. Marital status proof

   • If married, provide marriage certificate copy.

   • If divorced, provide divorced certificate copy.

8. Spouse passport copy, if married

9. Employment letter

   • Self declaration will not be accepted

   • Latest employment letter is must

   • In case of not having employment letter, you can provide company offer letter under your name along with current pay stub

   • In case the person is unemployed. On the application form under occupation section choose others and mention Unemployed. Next, give a self-declaration explaining why you are jobless.

   • If business / Self employed. Applicant need to attach article of incorporation.

10. Address proof

    • Driving license/utility bill/Notice of assessment/ Photo ID

11. Checklist

12. (2' x 2') one photograph.
