Documents required for LOST passport?

  1. Duly filled application form

  2. Two Photographs (2'x2')

  3. Annexure E

  4. Annexure F

  5. Police occurrence report (long report) ; The report should contain your last passport number.

  6. First Self declaration: In-case of Change of Signature/appearance, If the appearance and signature is same as per the Indian passport then no declaration is required.

    • Change of appearance required only if:

      • Turban to Non-turban, Hair to bald look and Clean shave to long beard.

  7. Second Self Declaration: The detail explanation to the Consulate, how the passport got lost.

  8. Indian Passport First and Last Page copy

  9. Status in Canada

    • Study Permit/Work permit/PR/Visitor

    • If the aforementioned status expires, then attach the extension letter

    • Incase of PR, landing paper is mandatory

  10. Marriage Certificate (In-case of Married)

  11. Address Proof in Canada and India

    • Driving License/Photo ID/Notice of assessment/Sin Number/Lease Document/Utility bill

Fees:

  1. Incase of Postal, the fees should be paid in form of money order or demand draft.

    • It should be favour of BLS International Services Canada : 206.70
