Lost your Indian passport while staying in Canada? Don’t panic—this guide is here to help. If you’re an Indian citizen over 18 who has misplaced, lost, or had your passport stolen, it’s important to act quickly and follow the correct reissue process. In this 2025 step-by-step guide, we explain how adults can apply for a lost Indian passport reissue in Canada, including FIR/reporting requirements, mandatory documents, affidavit formats, and how to file under normal or Tatkal processing. Ensure your application is complete and compliant to avoid delays.

Documents required for LOST passport application:

  1. Application Form - Duly filled and signed (Click Here)

  2. Annexure E - Duly filled and signed

  3. Annexure F - Duly filled and signed

  4. Police Occurrence Report Copy – Must include the passport number.

  5. Attach a self-explanatory letter to the Consulate describing when, where, and how the passport was lost.

  6. Indian Passport First and Last Page copy

  7. Status in Canada (Study permit/Work permit/Visitor Status/PR card with landing paper)

    • In-case of expired permit, attach the extension letter

  8. Marital Status Documentation

    • If Married

      1. Attach a copy of your Marriage Certificate.

      2. Only required if your current passport does not mention your spouse’s name (usually on the last page).

      3. Attach a copy of your spouse’s current passport (first and last page).

        1. Note: Spouse’s passport copy is required only if your passport does not contain their name.

    • If Divorced

      1. Attach a copy of your Divorce Certificate as official proof of marital status

  9. Indian address proof copy (Indian driver license/aadhar card/voter id/any relative address proof with declaration)

    • In-case your Indian address matches with your last page of your Indian passport then no separate Indian address proof is required.

  10. Canadian address proof copy (Driving License//Government Issued Photo ID/Utility Bill)

  13. Photograph (2x2)

    • Two recent passport-size photographs with white background.

    • One photograph should be pasted on the application form.

    • Size – 2 inch x 2 inch(51 mm x 51 mm)
      ii) Background –Background should be plain white colored without borders
      iii) Clothes – Dark colour
      iv) Eyes – Open
      v) Shoulders visible

  • Please note that applications for lost passports are not eligible for the Tatkal process and will be processed only under the normal scheme.
