Documents required for LOST passport?

2. Annexure E

3. Annexure F

4. First Self declaration: In-case of Change of Signature/appearance, If the appearance and signature is same as per the Indian passport then no declaration is required.

   • Change of appearance required only if:

     • Turban to Non-turban, Hair to bald look and Clean shave to long beard.

5. Second Self Declaration: The detail explanation to the Consulate, how the passport got damage.

6. Indian Passport First and Last Page copy

7. Status in Canada

   • Study Permit/Work permit/PR/Visitor

   • If the aforementioned status expires, then attach the extension letter

   • Incase of PR, landing paper is mandatory

8. Marriage Certificate (In-case of Married)

   • In-case not having marriage certificate, joint declaration of marriage is required.

   • For Waqf board marriages, a joint declaration is mandatory.

9. Address Proof in Canada and India

   • Driving License/Photo ID/Notice of assessment/Sin Number/Lease Document/Utility bill

Application Fees:

1. Incase of Postal, the fees should be paid in form of money order or demand draft.

   • It should be favour of BLS International Services Canada : 206.70
